Check exact medications, formulary tiers, preferred pharmacies, prior authorization, step therapy, quantities, exceptions, and annual changes.

A Medicare drug plan’s formulary is its list of covered prescription drugs. Medicare.gov explains that each plan has its own formulary, and coverage details can differ even when two plans list the same drug. A reliable comparison uses the exact medication, dosage, quantity, and pharmacy.

Build an exact medication list

  • Drug name, including brand or generic
  • Strength and dosage form
  • Quantity and refill frequency
  • Reasonable alternatives discussed with the prescriber
  • Vaccines or specialty medications
  • Preferred retail, mail-order, or specialty pharmacy

Check more than covered or not covered

Find the formulary tier, deductible treatment, copay or coinsurance, preferred-pharmacy pricing, and whether the drug uses prior authorization, step therapy, or a quantity limit. A plan may cover a drug only under specific clinical or administrative conditions.

Pharmacy status matters

A pharmacy can be in network without being preferred. Costs can differ between standard and preferred network pharmacies, and mail-order is not always the lowest option. Confirm the exact location and pharmacy type, especially for specialty drugs or long-term supplies.

Formularies can change

Medicare drug plans can make formulary changes under Medicare rules. Review the Annual Notice of Change, Evidence of Coverage, current formulary, and plan notices. Do not assume that last year’s coverage or pharmacy arrangement continues unchanged.

Exceptions and appeals

When a drug is not listed, is subject to a coverage rule, or is placed on a nonpreferred tier, the enrollee or prescriber may be able to request an exception under plan and Medicare procedures. The prescriber generally must support the medical reason. Follow the plan’s current instructions and urgent-request process when applicable.

  • Run every medication through the official Medicare Plan Finder and plan formulary
  • Confirm pharmacies and utilization rules directly with the plan
  • Compare annual premium plus expected drug costs, not premium alone
  • Review whether Part B rather than Part D covers a medication in a particular setting
  • Recheck during the Annual Enrollment Period and after plan notices
